The Clinical Pharmacist - Anticoagulation is responsible for evaluating and monitoring patient medications for safety, efficacy, and cost-effectiveness while providing medication administration, patient education, and drug information to healthcare teams. This role collaborates with clinical pharmacy leadership to improve quality of care and supports professional development. The position requires a pharmacy degree and appropriate licensure, with work primarily conducted onsite at Providence St. Peter Hospital.

The Clinical Pharmacist is responsible for assessing patient specific medications and clinical data in order to evaluate & monitor for safety, efficacy and pharmaco-economic optimization opportunities. In this role, the pharmacist may administer medications, monitors clinical data, educates patients, and provides general drug information to the healthcare team. This role works closely with the Clinical Pharmacy leadership thru various projects aimed at improving the quality of care and personal professional growth and development.
