SUMMARY: Provides screening, assessment, evidence-based treatment, and care coordination services. The position is part of a multi-disciplinary team (e.g., ACT, IHH, Healthy Transitions, CFTO) that is designed to provide outreach, comprehensive evidence-based treatment, referral, and support services for a designated population struggling with severe mental illness and co-occurring substance use and their family members. The goals of the Substance Use Counselor are: 1) to assist clients with substance use recovery; 2) identify and provide intervention to overcome barriers to client recovery from substance use; and 3) promote, improve and sustain the overall well-being of the target population. Contributes information to the person-centered treatment planning process. Creates rehabilitation activities for clients to help them develop independent living skills and promote social and recreational opportunities. Uses Motivational Interviewing and Cognitive Behavior Therapy to engage and treat clients, and other evidence-based practices as identified for targeted populations. Meets with multidisciplinary team members, contributing knowledge and expertise in treatment planning, case review, and interventions Supports clients as they work towards their substance use recovery goals Maintains frequent communication and collaboration with client, team members, and community providers, and partners. Maintains knowledge of available providers and resources in the community to address specialized needs. Participates in supervision, team conferencing, department and agency meetings. Maintains detailed, accurate, and timely documentation in medical record. Collects, records, and analyzes required data within scope of job responsibilities. Enhances and maintains professional expertise through educational activities and trainings. Engages in trainings on evidence-based practice models specific to their assigned population, including motivational interviewing and cognitive behavioral therapy, implementing learned skills in daily interventions. Meets requirements for client contact time according to treatment plan, and for agency productivity targets. Follows agency policies and standards for client rights and confidentiality. Other duties as assigned MINIMUM QUALIFICATIONS: QUALIFICATIONS: Bachelor’s Degree in human services or related field preferred, LCDP or ability to obtain LCDP within one year required. Two years’ experience working in a community based behavioral health setting preferred; experience in substance use and co-occurring disorders and the implementation of evidence-based practices preferred. Ability to obtain LCDP within one year is acceptable JOB KNOWLEDGE, SKILL, AND ABILITY: Valid driver's license and use of car during normal working hours, or other means of transportation to complete major functions of job. Automobile liability insurance coverage for vehicle used in accordance with center requirements. Understanding of mental health and substance use issues Understanding of human development and family systems model of care Oral and written communication skills at a proficient level, ensuring effective and professional communication Ability to provide crisis intervention, brief counseling, or case management when determined necessary. Diplomacy, strong collaboration and care coordination skills Report writing abilities, to include the formulation of clinical assessments and progress reports Ability to understand, embrace and reinforce agency ethics and codes of conduct in all contacts with clients, coworkers and the community
Pay Range $47,143.10-$77,818.83
