MHALA also provides training programs for individuals joining the behavioral health workforce and for the larger community around mental health issues.

Personal Service Coordinator

35 hours per week / $24.00 per hour

(The actual compensation will be determined based on experience and other factors permitted by law.)

Summary: Serves as the primary support and case manager for adults with mental illness by assisting them to live as independently as possible in their chosen community.

Essential Duties and Responsibilities : This is not an exhaustive task list, and other duties may be assigned as deemed appropriate or necessary by the supervisor or program director.   • Develops a Recovery Plan and/or Personal Service Plan with each member that identifies his/her personal needs and goals.
  • Mediates and advocates on behalf of the member to help him/her obtain and keep housing, health care benefits, entitlements, etc.
  • Visits members in their place of residence, hospital or jail on an as-needed basis, in order to help them live successfully and to prevent them from becoming homeless.
  • Transports members to appointments and on errands when appropriate.
  • Oversees and monitors members’ individual and group activities both inside the program and out in the community.
  • Provides coaching, counseling, mentoring, instruction, training, feedback and positive reinforcement for members in areas such as money management, grooming and hygiene, meal preparation, public transportation and other independent living skills.
  • Devises and implements strategies for outreach to members who tend to isolate themselves.
  • Knowledgeable and practices in motivational interviewing and harm reduction.
  • Supports individuals and team goals in community integration and employment.
  • Assists members in achieving community integration and employment goals.
  • Participates in staff and community meetings and in the day-to-day operations of the program as needed.
  • Required to bill 85 hours per month into the Electronic health care System.
  • Maintains all records and documentation needed for program compliance and accountability in accordance with MHALA standards and funding source requirements.

Other position requirements:

  • High School Diploma or GED, plus 2 years’ experience providing psychosocial rehab related services to a disabled population, preferred.
  • All billing and supporting documentation shall be entered into MHALA’s EHR (electronic health record system) within 24 hours from the date of service delivery.
  • All required data collection documents (as applicable to your program: MORS, Determinants, KEC, IHOMS, HMIS, etc.) are submitted on or before due date.
  • Demonstrate knowledge of community resources.
  • Must function well in a team-oriented approach to member services.
  • Must have a valid CPR certificate.
  • Knowledgeable in computer applications such as Microsoft Word, Excel and Outlook.
  • Flexible work schedule to include some evenings or weekends, and on call cell phone services when necessary.
  • Driving with current class C driver’s license, a vehicle for transportation, proof of valid automobile insurance, the ability to be insured by MHALA’s liability insurance are conditions of employment
  • TB clearance and re-testing every year are conditions of continued employment
  • COVID-19 vaccinations or booster shots are not currently required, however this may become a requirement in the future. Failure to abide by any agency policy, including those relating to a vaccine or booster shot may result in termination of employment
  • Employment verification directly from three (3) past employers within the most recent 7-year period
